Direct bill: Workers Compensation, Auto Accidents

Dr. Herold is out of network with all private insurance companies.

If you are interested in billing insurance please call your insurance company to make sure you have out-of-network chiropractic coverage.

We may be able to bill directly, or have you pay out of pocket and provide you with a superbill/receipt for direct reimbursement from your insurance company. 

Medicare: Only spinal adjustments under 'active' care are covered, no maintenance or preventive coverage. Initial exam, therapies and all other services that are not a spinal adjustments are out of pocket.  (In most situations it is cheaper to go with Dr. Herold's self pay discount through Patient Options membership than to bill Medicare for services.)

Dr. Herold cannot accept any form of OHP/Medicaid, BlueCross BlueShield, MODA, Aetna or Kaiser or any plans managed by the ASH network.  (Superbills can still be provided for patient reimbursement if you have out of network coverage.)
